About this policy

This policy explains how Citymed Compounding Pharmacy collects, uses, stores and shares your personal and health information, including information you send through the enquiry form on this website.

What information we collect

When you send us a compounding enquiry through this website, we collect:

A prescription contains health information, such as the medicine, dose and prescriber, and may show other details such as your address or a Medicare or concession card number.

When you visit or call the pharmacy, we also collect the information we need to dispense your medication and advise you safely.

Who we share it with

We do not sell your information. We share it only where needed to care for you or where the law requires:

Service providers and overseas disclosure

Some of the technology we use to run the website and receive enquiries is based overseas, mainly in the United States. Your enquiry may be handled by these providers:

By ticking the consent box and submitting the form, you agree to your information being disclosed to these providers. We choose providers that have their own security and privacy commitments, but we cannot control how overseas providers operate.

Email is not a fully secure way to send sensitive information. If you would rather not use the form, you can phone us on (02) 9235 0406 or bring your prescription into the pharmacy.

Storage, security and how long we keep it

Enquiry emails are kept for 2 years, then securely deleted. Access is limited to Citymed pharmacy staff, who are all pharmacists or pharmacy students trained in handling patient information. Enquiries are normally actioned by a pharmacist. The website uses an encrypted (HTTPS) connection, so your form submission is protected on its way to us.

We may keep dispensing and other records for longer where the law requires. We take reasonable steps to protect information from misuse, loss and unauthorised access.

Access and correction

You can ask to see the information we hold about you, or ask us to correct it, by contacting us on (02) 9235 0406 or citymedcompounding@gmail.com. We may ask you to confirm your identity first, and we will respond within a reasonable time, normally within 30 days.
